Danshen is the dried root of a red-flowered sage that grows across China and neighbouring regions. For two thousand years it has been used in Chinese herbal practice for problems involving the heart and circulation, and it is among the best-selling herbal products in Asia. Its appeal outside Asia rests on two families of active substances — one that dissolves in water, one that dissolves in oil — which together appear to relax blood vessels, quiet inflammation, and make platelets less prone to clumping.

The research base is unusually large in volume: thousands of trials have been run in China, most of them adding the root to standard heart medicines rather than replacing them, and a commercial drug-development programme has spent more than two decades seeking formal approval in the United States for a Danshen-based tablet for chest pain. The quality of that literature, however, varies widely, and a European trial of a plain water extract reported no benefit at all.

## Mechanism of Action

Danshen root contains two chemically opposite groups of actives. The water-soluble phenolic acids — tanshinol (danshensu), salvianolic acid B, and rosmarinic acid — dominate aqueous extracts. The oil-soluble diterpene quinones — tanshinone IIA, tanshinone I, and cryptotanshinone — dominate alcohol extracts and give the root its colour.

Both groups converge on the vessel wall. Phenolic acids raise nitric oxide (a gas the vessel lining makes to relax blood vessels) by activating endothelial nitric oxide synthase (eNOS — the enzyme that produces it), scavenge free radicals, and suppress NF-κB (a master switch that turns on inflammatory genes). Tanshinones block calcium entry into heart and smooth-muscle cells, activate Nrf2 (a switch that turns on the cell's own antioxidant genes), and inhibit platelet aggregation, the basis of the [antithrombotic effect](https://pubmed.ncbi.nlm.nih.gov/11302416/).

Pharmacologically, tanshinol has a short elimination half-life of about 0.5 hours and is [cleared largely unchanged in urine](https://pubmed.ncbi.nlm.nih.gov/18474682/), while the lipophilic tanshinones distribute into tissue but have very poor oral bioavailability — [micronising the root raised tanshinone exposure five- to several-hundred-fold](https://pubmed.ncbi.nlm.nih.gov/28680091/). Danshen appears in the *Shennong Bencao Jing*, compiled around the first to second century CE, as a superior-grade herb used to "invigorate blood and dispel stasis". Its classical indications were menstrual disorders, abdominal masses, restlessness and insomnia, and sores — not chest pain, which entered the picture much later once stasis theory was mapped onto cardiac ischaemia.

The modern era began in 1930s–1970s China with the isolation of the tanshinones and the development of an injectable extract for acute cardiac and cerebral ischaemia. Tasly's Compound Danshen Dripping Pill, combining Danshen with *Panax notoginseng* and borneol, was launched in 1993 and became the country's best-selling herbal cardiac product. It was the first Chinese herbal medicine accepted into the United States drug-approval pathway, clearing a [Western dose-escalation tolerability study](https://pubmed.ncbi.nlm.nih.gov/23232790/) before entering late-stage trials.

Western replication has been mixed rather than simply negative. Danshen preparations added to conventional antianginal drugs reduce the frequency and severity of exertional chest pain and improve ischaemic changes on the electrocardiogram (ECG — a recording of the heart's electrical activity). * **Coronary microcirculation after stenting:** [NCT07621107](https://clinicaltrials.gov/study/NCT07621107) is a phase 2 trial of a Danshen-containing tablet in 70 patients, measuring microvascular function after emergency stenting for myocardial infarction.

## Conclusion

Danshen is a traditional Chinese root remedy whose case rests on a very large but uneven body of clinical research. The most consistent signal is symptom relief in stable chest pain when the root is added to standard heart medicines, together with smaller changes in blood pressure, blood thickness, kidney and eye measures in diabetes, thinking scores after impaired brain blood flow, and urinary symptoms in men. Almost all of that work comes from Chinese hospitals and from the companies that make and sell the products, and the trials are typically small, briefly reported, and short. The one carefully controlled European study, using a simple water extract, found nothing at all. Direction of effect repeats often enough to be interesting; size of effect, and any change in survival or heart attacks, remain unsettled.

The main hazard is bleeding. The root thins the blood on its own and, in published cases, has pushed people already taking blood-thinning drugs into dangerous over-thinning. It can also distort a common heart-drug blood test, add to the effect of blood-pressure medicines, and cause stomach upset or dizziness. What is in the bottle varies widely between brands, so a product may not match what was studied.

Taken together, the picture is of a cheap and generally well-tolerated add-on with one genuine interaction hazard, plausible short-term effects on symptoms and blood-flow measures, and an evidence base whose funding and quality invite caution rather than confidence.
