Red yeast rice is a fermented food supplement taken to lower cholesterol without a prescription; its active compound is the same molecule as a prescription cholesterol-lowering medication. Cholesterol lowering is reliable, with signals for fewer repeat heart events, lower inflammation and healthier artery walls. The dominant problem is product quality: active content ranges from almost nothing to more than a prescription dose, and labels rarely state it. (Full Review)
| Marker | Target | Why |
|---|---|---|
| Apolipoprotein B | Below 80 mg/dL; below 60 mg/dL if established atherosclerosis | Counts every plaque-forming particle; the primary success measure |
| LDL cholesterol | Below 100 mg/dL; below 70 mg/dL if established atherosclerosis | The endpoint used in every trial, so the only comparable measure |
| Lipoprotein(a) | Below 30 mg/dL or below 75 nmol/L | Genetically fixed risk not lowered by this intervention; reframes the target |
| Alanine aminotransferase | 10–25 U/L in men, 10–20 U/L in women | Earliest marker of drug-related liver injury |
| Creatine kinase | No established optimal target; track change from own baseline | Detects subclinical muscle injury before symptoms |
| Estimated glomerular filtration rate | Above 90 ml/min/1.73m² | Detects the tubular kidney injury seen with contaminated product |
| Urine glucose with normal blood glucose | Negative | Earliest sign of Fanconi-type tubular injury, preceding filtration loss |
| High-sensitivity C-reactive protein | Below 1.0 mg/L | Tracks the inflammatory component of the response |
| Haemoglobin A1c | 5.0–5.4% | Screens for the glucose worsening seen with the statin class |
| Coenzyme Q10 | No established optimal target; track change from own baseline | Assesses depletion of the shared synthesis pathway |
Cadence: Full panel at baseline; lipids, liver enzymes, creatine kinase and kidney function repeated at 12 weeks. If values are stable, every 6 months for the first year, then every 6–12 months indefinitely. Any change of product, batch or brand resets the schedule. Haemoglobin A1c annually; lipoprotein(a) once in a lifetime.
