Amplified cGMP signaling promotes glucose uptake and mitochondrial biogenesis in adipose and muscle tissue in preclinical models, and one randomized trial of sildenafil in prediabetes reported improved insulin sensitivity. The direct tadalafil evidence does not support it: Effect of tadalafil administration on insulin secretion and insulin sensitivity in obese men (González-Ortiz et al., 2017) randomized 18 obese non-diabetic men to 5 mg nightly for 28 days and found no change in insulin secretion or sensitivity. The basis for including it at all is mechanistic and cross-agent, and an active randomized trial is currently testing the question directly.

Immune Modulation in Cancer

Phosphodiesterase type 5 inhibition suppresses myeloid-derived suppressor cells (immature immune cells that tumors recruit to shut down T-cell attack), and small early-phase studies in head and neck cancer have reported restored T-cell responses. Whether this translates into any survival or recurrence benefit is entirely unknown, and observational data on cancer incidence in users are dominated by detection and confounding artifacts. The basis is mechanistic and limited to small uncontrolled or early-phase studies.

Benefit-Modifying Factors

  • Nitric oxide synthase genotype. Tadalafil amplifies an existing nitric oxide signal rather than generating one, so response depends on how much signal the vessel lining still produces. Variants in NOS3 (the gene encoding endothelial nitric oxide synthase, the enzyme that makes nitric oxide in vessel lining cells) alter response magnitude: Endothelial nitric oxide synthase genotypes and haplotypes modify the responses to sildenafil in patients with erectile dysfunction (Muniz et al., 2013) found genotype-dependent differences in response within the same class, and analogous findings exist for neuronal nitric oxide synthase variants.

  • CD36 and GNB3 variants. A Common CD36 Variant Influences Endothelial Function and Response to Treatment with Phosphodiesterase 5 Inhibition (Shibao et al., 2016) showed that carriers of a common variant in CD36 (a gene encoding a fatty-acid transport receptor that also regulates nitric oxide availability) had both worse baseline endothelial function and a different treatment response. Variants in GNB3 (a gene encoding a signal-relay protein subunit that sits downstream of many cell-surface receptors) have likewise been associated with non-response in both erectile and pulmonary hypertension settings.

  • Baseline endothelial function and blood pressure. The lower the starting flow-mediated dilation and the higher the starting blood pressure, the larger the measurable improvement — this is a consistent pattern across the vascular-parameter trials, and it means that a metabolically healthy person with already-normal arterial function has the least room to gain. Baseline testosterone matters in the opposite direction: hypogonadal men (men whose testes produce too little testosterone) respond poorly to phosphodiesterase type 5 inhibitors until testosterone is restored, because androgen signaling maintains the nitric oxide synthase machinery in the erectile tissue.

  • Sex. Almost the entire benefit literature is in men. Trials of the class in female sexual dysfunction have produced small and inconsistent effects, with the clearest signal confined to women whose dysfunction is drug-induced. Women were represented in the pulmonary hypertension and small-vessel-disease trials, where efficacy appeared comparable, but the ETLAS-2 trial found that women tolerated 20 mg daily far less well than men, which limits achievable exposure and therefore achievable benefit.

  • Pre-existing conditions. Diabetes, established atherosclerosis and prior pelvic surgery or radiation all reduce erectile response, because they damage the nerve and vessel substrate the drug depends on. Conversely, the observational mortality signal is largest in exactly these higher-risk groups — Phosphodiesterase type-5 inhibitor use in type 2 diabetes is associated with a reduction in all-cause mortality (Anderson et al., 2016) followed 5,956 men with type 2 diabetes for 7.5 years and found the largest advantage among those with prior myocardial infarction, which is also where confounding by indication is hardest to exclude.

  • Age. Efficacy on erectile and urinary endpoints is preserved into the eighth decade, and the vascular-parameter improvements are if anything larger in older arteries with more to correct. Working against this, liver CYP3A4 activity and baroreflex buffering (the automatic heart-rate and vessel-tone response that stabilizes blood pressure when posture changes) both decline with age, so older users reach higher plasma concentrations and tolerate the blood-pressure drop less well — the practical effect at the older end of the target range is that the maximum tolerated dose, and therefore the achievable benefit, is often lower.

Potential Risks & Side Effects

Headache

Headache is the single most common adverse effect, arising from dilation of blood vessels inside the skull and in the membranes covering the brain — the same mechanism responsible for nitrate headache. It is documented in every registration trial and in the pooled randomized literature, is dose-dependent, and usually attenuates over the first two to three weeks of continuous dosing. It is typically mild to moderate and fully reversible on withdrawal, and it is substantially less frequent on the low daily doses used for continuous vascular purposes than on the high on-demand doses used for erectile purposes.

Magnitude: 11–15% incidence at on-demand doses of 10–20 mg versus roughly 5% on placebo; 3–6% at 2.5–5 mg once daily.

Dyspepsia and Reflux

Tadalafil relaxes the lower esophageal sphincter and gastric smooth muscle through the same cGMP mechanism, producing indigestion, heartburn and acid reflux. This is well documented in the trial literature and in a systematic review of phosphodiesterase type 5 inhibitor effects on esophageal motility, which confirmed measurable reductions in sphincter pressure. It is dose-dependent and reversible, but unlike headache it does not reliably attenuate with continued use, and it is a leading reason for discontinuation in people with pre-existing reflux.

Magnitude: 4–10% incidence at on-demand doses versus 1–2% on placebo; 4–5% at 2.5–5 mg once daily.

Back Pain and Myalgia

Myalgia (muscle ache) affecting the lower back, buttocks and thighs, typically beginning 12–24 hours after a dose and lasting up to two days, is tadalafil’s signature adverse effect and is not shared by sildenafil or vardenafil. The accepted explanation is tadalafil’s weak selectivity over PDE11, an enzyme concentrated in skeletal muscle, against which it is only about 9- to 14-fold less potent than against PDE5. It is benign, reversible and does not involve muscle enzyme elevation, but it is the most common cause of switching away from tadalafil to another agent in the class.

Magnitude: Back pain in 2–6% and myalgia in 1–4% at on-demand doses versus roughly 1% on placebo; 2–4% at 2.5–5 mg once daily.

Severe Hypotension with Nitrates or Riociguat

Combining tadalafil with any organic nitrate, nitrite or the soluble guanylate cyclase stimulator riociguat (a drug that switches on the same vessel-relaxing enzyme from a different point in the pathway) produces profound, potentially fatal hypotension (dangerously low blood pressure), because both agents act on the same nitric oxide–cGMP pathway at different points and their effects multiply rather than add. This is an absolute contraindication in the prescribing information for every product in the class and is supported by controlled interaction studies and by fatal case reports. Tadalafil’s long half-life makes this more dangerous than for shorter-acting agents: the interaction window extends at least 48 hours after the last dose, which is a genuine emergency-care hazard if chest pain develops.

Magnitude: Mean additional systolic blood pressure fall of 25–50 mmHg in controlled co-administration studies, with individual falls exceeding 80 mmHg and documented fatalities.

Symptomatic Hypotension and Dizziness

Beyond the nitrate interaction, tadalafil lowers blood pressure on its own by roughly 3 mmHg systolic, and this becomes clinically relevant when stacked with alpha-blockers, multiple antihypertensives, diuretics, alcohol, dehydration or autonomic neuropathy (damage to the nerves that automatically regulate blood pressure and heart rate). Randomized data confirm the blood-pressure effect, and postural dizziness and syncope (fainting) appear consistently in post-marketing reports. It is dose-dependent and reversible, and it is more consequential in older users and in anyone whose blood pressure is already well controlled or on the low side — precisely the profile of many people who would take this drug for longevity purposes.

Magnitude: Systolic blood pressure −2.80 mmHg and diastolic −1.80 mmHg as a class average from randomized trials; dizziness reported in 1–2% of trial participants, higher with concurrent alpha-blockers.

Discontinuation Due to Adverse Effects

The cumulative burden of headache, indigestion, back ache and nasal congestion causes a meaningful fraction of users to stop, even at low daily doses. Wang et al., 2018 quantified this at the 5 mg once-daily dose across thirteen placebo-controlled trials. Real-world persistence is worse than trial persistence, and the ETLAS-2 trial in older stroke patients found that at 20 mg daily only 68% of the tadalafil group reached 90% compliance versus 94% on placebo, with adverse events in 76% versus 36%. This is a practical rather than a medical harm, but it is the single most likely reason a longevity-motivated user will not sustain the exposure the observational data are based on.

Magnitude: Odds ratio (a ratio of the odds of an event in the two groups; above 1 means the event is more frequent on the drug) 1.79 (95% confidence interval 1.12–2.85) for discontinuation due to adverse events at 5 mg daily versus placebo; odds ratio 5.49 (1.81–18.07) for any adverse event at 20 mg daily in older patients.

Nasal Congestion and Flushing

Dilation of the nasal mucosal vasculature produces nasal obstruction and rhinitis, and cutaneous vasodilation produces facial flushing. Both are direct extensions of the drug’s intended action, are documented across the trial literature, and are dose-dependent and fully reversible. Nasal congestion deserves separate mention from the cosmetic issue of flushing because sustained nocturnal congestion promotes mouth breathing and can worsen snoring and the symptom burden of sleep-disordered breathing in susceptible users, which is directly counterproductive for anyone dosing at bedtime for longevity reasons.

Magnitude: Nasal congestion in 2–4% and flushing in 1–3% at on-demand doses versus roughly 1% on placebo.

Low 🟥

Non-Arteritic Anterior Ischemic Optic Neuropathy

NAION (non-arteritic anterior ischemic optic neuropathy) is a sudden, usually painless and typically permanent loss of part of the visual field in one eye, caused by an infarction of the optic nerve head. It appears in the prescribing information for the whole class following post-marketing reports, and Risk of Ocular Adverse Events Associated With Use of Phosphodiesterase 5 Inhibitors in Men in the US (Etminan et al., 2022) found an elevated rate of a composite of serious ocular events among regular users in a large claims cohort. Causality remains contested because the drug is prescribed to men who already carry the vascular risk factors for the condition; the strongest independent predictor is an anatomically crowded optic disc, and prior NAION in one eye is treated as a contraindication.

Magnitude: Background incidence 2.5–11.8 cases per 100,000 men aged 50 or over per year, with roughly a doubling of relative risk among regular users — an absolute excess on the order of a few cases per 100,000 person-years.

Sudden Sensorineural Hearing Loss

Abrupt, usually unilateral hearing loss, sometimes with tinnitus or vertigo, has been reported after phosphodiesterase type 5 inhibitor use and prompted an FDA labeling change for the class in 2007. The proposed mechanism is altered cochlear microcirculation or direct cGMP effects on cochlear hair cells. A prospective audiometric study of tadalafil specifically found measurable but subclinical high-frequency threshold shifts. Recovery is partial at best, and the event is rare enough that a causal contribution cannot be separated from background incidence with confidence.

Magnitude: Fewer than 100 case reports across the entire class worldwide since 1998 against hundreds of millions of exposures; measurable subclinical threshold shifts of a few decibels at high frequencies in prospective testing.

Priapism and Prolonged Erection

An erection lasting more than four hours is a urological emergency that can cause irreversible fibrosis and permanent erectile loss if untreated. It is rare with tadalafil monotherapy, and the risk concentrates in men with sickle cell disease, multiple myeloma, leukemia, penile anatomical abnormality, or concurrent use of intracavernosal injections (drugs injected directly into the erectile tissue) or alpha-blocking psychiatric medications. It appears in the prescribing information and in isolated case reports rather than in trial data, since trials excluded the predisposing conditions.

Magnitude: Reported at well under 0.1% in clinical trials and post-marketing surveillance for the class; near-zero with monotherapy in the absence of a predisposing condition.

Hypersensitivity Reactions

Immune reactions to tadalafil range from rash and urticaria (raised, intensely itchy welts, commonly called hives) through angioedema (rapid deep swelling of the lips, tongue, throat or face that can obstruct breathing) to the severe blistering skin reactions Stevens-Johnson syndrome and exfoliative dermatitis (widespread peeling of the skin). They appear in the prescribing information for the whole class on the basis of post-marketing reports rather than trial data, and no mechanism specific to phosphodiesterase inhibition has been established — the reactions are attributed to the molecule itself or to tablet ingredients rather than to cGMP signaling. Severity spans the trivial to the life-threatening and does not predict itself: a mild rash on one exposure does not guarantee a mild reaction on the next, which is why this is the one part of the tadalafil safety profile where continuing after the first sign is genuinely hazardous.

Magnitude: Reported at well under 0.1% across clinical trials and post-marketing surveillance for the class; the severe blistering forms exist only as isolated case reports against hundreds of millions of exposures.

Melanoma Association ⚠️ Conflicted

Several cohort studies report a modestly higher incidence of melanoma among users of this drug class, which drew attention because the same cGMP pathway is downstream of BRAF (a gene that drives cell-growth signals and is frequently mutated in melanoma). Phosphodiesterase type 5 inhibitors and risk of melanoma: A meta-analysis (Tang et al., 2017) confirmed a small pooled association. The evidence is conflicted because the same datasets show a comparable increase in basal cell carcinoma — a tumor with no plausible link to this pathway but a strong link to sun exposure and to skin surveillance — and because no dose–response gradient is observed. The consensus reading of the meta-analyses is that the association reflects confounding by sun exposure and detection bias rather than a drug effect, but this has not been definitively resolved.

Magnitude: Pooled relative risk approximately 1.11–1.12 (95% confidence interval roughly 1.02–1.23), with a similar-magnitude association for basal cell carcinoma.

Speculative 🟨

Long-Term Consequences of Continuous Pathway Suppression

Every human trial of continuous daily dosing has run for weeks to months; nobody has studied what decades of uninterrupted PDE5 blockade does to receptor density, enzyme expression or the responsiveness of the nitric oxide system. Downregulation or compensatory upregulation of PDE5 with chronic inhibition is biologically plausible and has been observed for other chronically inhibited enzymes, and would predict both tolerance and a rebound on withdrawal. There are no controlled human data on this at all; the concern is mechanistic only, and the available short-term evidence points the other way, with benefit persisting for weeks after discontinuation.

Masking of Progressive Vascular Disease

Erectile difficulty is one of the earliest clinical manifestations of systemic endothelial dysfunction, typically preceding a coronary event by around three years. Pharmacologically restoring function removes that warning signal without treating the underlying atherosclerosis, so a person could plausibly lose the prompt that would otherwise have driven them to cardiovascular evaluation. This is a reasoning-based concern rather than an observed harm — no study has documented delayed diagnosis attributable to phosphodiesterase type 5 inhibitor use, and the observational cardiovascular data point in the opposite direction.

Blunting of Exercise-Induced Vascular Adaptation

Training-induced improvements in vessel function are driven in part by repeated shear-stress signaling, and pharmacologically pre-dilating arteries could in principle reduce the shear stimulus and therefore the adaptation, in the way that some antioxidants blunt training responses. No study has tested whether tadalafil interferes with vascular or mitochondrial training adaptations. The basis is entirely mechanistic reasoning by analogy, and the opposing argument — that better perfusion permits greater training volume — is equally untested.

Risk-Modifying Factors

  • CYP3A4 activity and genotype. Tadalafil clearance depends almost entirely on CYP3A4, so anything that reduces this enzyme’s activity raises exposure and therefore side-effect intensity. Genetic variability in CYP3A4 and CYP3A5 (a closely related liver enzyme that shares many of the same drug substrates) expression is substantial between individuals, and CYP3A5 expresser status differs markedly by ancestry — this is the most likely explanation for why some people find 5 mg intolerable while others take 20 mg without complaint. There is no clinically validated genotype test used to guide dosing; the practical proxy is empirical dose titration.

  • PDE11 sensitivity. The back-pain and myalgia effect is idiosyncratic in a way the other adverse effects are not: some people get it reliably at 5 mg and others never get it at 20 mg. Variation in PDE11A expression or affinity is the proposed explanation, and PDE11A loss-of-function variants are known in humans, though no study has linked them to tadalafil tolerability. Anyone who experiences it can usually eliminate it by switching to sildenafil, which lacks the PDE11 activity.

  • Baseline blood pressure and autonomic function. A person whose seated systolic pressure is already in the low 100s, or who has orthostatic hypotension (a fall in blood pressure on standing), autonomic neuropathy from diabetes, or is volume-depleted from a low-carbohydrate diet or sauna use, has far less reserve to absorb a further 3 mmHg drop plus the acute post-dose effect. Baseline hematocrit (the proportion of blood volume made up of red cells) and sodium status modify this in the same direction.

  • Sex. The safety literature is overwhelmingly male, and the one randomized trial that dosed both sexes at 20 mg daily found a striking asymmetry in tolerability: 46% of women discontinued versus 16% of men in ETLAS-2. Women were not systematically studied at the lower doses. Tadalafil is not indicated in women other than for pulmonary arterial hypertension, and it is contraindicated in pregnancy planning contexts only insofar as data are absent rather than adverse.

  • Pre-existing conditions. Severe hepatic impairment (Child-Pugh Class C, the most advanced grade of liver dysfunction) removes the primary clearance route and once-daily dosing is not recommended. Severe renal impairment (creatinine clearance below 30 mL/min) increases exposure and caps on-demand dosing at 5 mg every 72 hours, with once-daily dosing not recommended. Retinitis pigmentosa (an inherited degeneration of the light-sensing layer of the eye), prior NAION, uncontrolled arrhythmia (abnormal heart rhythm), recent myocardial infarction (within 90 days), recent stroke (within 6 months), uncontrolled hypertension (above 170/100 mmHg), hypotension (below 90/50 mmHg), and NYHA (New York Heart Association) Class III–IV heart failure — the grades denoting marked limitation of, or symptoms at, minimal activity — all mark higher-risk groups excluded from the trials.

  • Age. Plasma exposure rises modestly with age from reduced liver clearance, and the baroreflex buffers blood-pressure swings less well, so both the frequency and the consequence of hypotensive events increase. The background incidence of the rare ocular and auditory events also rises steeply with age and with the vascular comorbidity that accompanies it, meaning that the same relative risk translates into a larger absolute excess for someone at 70 than at 45. Fall risk from post-dose dizziness is a genuine consideration at the older end of the target range that is absent at the younger end.

Key Interactions & Contraindications

  • Organic nitrates and nitric oxide donors — absolute contraindication. Nitroglycerin (including sublingual and patch forms), isosorbide mononitrate and dinitrate, sodium nitroprusside, and recreational amyl or alkyl nitrites (“poppers”) all produce profound, potentially fatal hypotension in combination. Mitigating action: a minimum 48-hour washout after the last tadalafil dose before any nitrate is given, and carrying documentation of tadalafil use so emergency clinicians do not administer nitrates for chest pain.

  • Riociguat and other soluble guanylate cyclase stimulators — absolute contraindication. Riociguat acts on the enzyme immediately upstream of the cGMP that tadalafil preserves; the clinical consequence is symptomatic hypotension and syncope. Mitigating action: 24 hours off riociguat before starting tadalafil, and 48 hours off tadalafil before starting riociguat.

  • Alpha-blockers — caution. Doxazosin, terazosin, alfuzosin, tamsulosin and silodosin all lower blood pressure by an independent mechanism; the consequence is orthostatic hypotension and syncope, most pronounced with the non-selective agents doxazosin and terazosin. Mitigating action: establish a stable alpha-blocker dose first, start tadalafil at the lowest dose, separate administration by at least four hours, and rise slowly for the first week. The tamsulosin plus tadalafil 5 mg combination has been studied extensively and is generally well tolerated.

  • Antihypertensives and diuretics — monitor. Angiotensin-converting enzyme inhibitors (lisinopril, ramipril, enalapril) and angiotensin receptor blockers (losartan, valsartan, telmisartan) — two families of blood-pressure drugs that block the hormone system driving vessel constriction — along with calcium channel blockers (amlodipine, nifedipine, diltiazem), which relax artery walls by limiting calcium entry, beta-blockers (metoprolol, bisoprolol, carvedilol), which slow the heart, and thiazide diuretics (hydrochlorothiazide, chlorthalidone, indapamide), which make the kidneys excrete more salt and water, all produce additive blood-pressure reduction; the consequence is dizziness and, rarely, syncope. Mitigating action: measure seated and standing blood pressure before starting and at two weeks, and consider dosing at bedtime so the nadir occurs while recumbent.

  • CYP3A4 inhibitors — caution with dose reduction. Ketoconazole, itraconazole, clarithromycin, erythromycin, ritonavir and other protease inhibitors (antiviral drugs used mainly against HIV), cobicistat, and grapefruit or Seville orange juice all raise tadalafil exposure, intensifying every dose-dependent side effect. Mitigating action: with potent inhibitors, cap on-demand dosing at 10 mg per 72 hours and once-daily dosing at 2.5 mg; avoid grapefruit juice entirely during continuous dosing.

  • CYP3A4 inducers — caution with efficacy loss. Rifampicin, carbamazepine, phenytoin, phenobarbital and St. John’s wort accelerate clearance; the consequence is loss of effect rather than toxicity. Mitigating action: expect reduced efficacy and avoid compensating with dose escalation without medical supervision, since induction reverses over weeks after the inducer is stopped.

  • Over-the-counter medications — monitor. Oral decongestants containing pseudoephedrine or phenylephrine oppose tadalafil’s vasodilation and raise blood pressure; non-steroidal anti-inflammatory drugs such as ibuprofen and naproxen blunt the antihypertensive effect through prostaglandin inhibition; over-the-counter cimetidine is a weak CYP3A4 inhibitor that modestly raises exposure. Antacids containing magnesium hydroxide and aluminum hydroxide slow tadalafil absorption without reducing total exposure. Mitigating action: separate antacids by two hours; no dose change is needed for the others.

  • Supplements with additive vasodilatory or blood-pressure-lowering effects — caution. L-Citrulline and L-Arginine (nitric oxide precursors), dietary nitrate sources such as beetroot juice, garlic extract, hawthorn, magnesium, taurine, coenzyme Q10, hibiscus, potassium and omega-3 fatty acids all lower blood pressure or increase nitric oxide availability; combined with tadalafil the consequence is additive hypotension and headache. Mitigating action: introduce one at a time, separate high-dose nitrate supplements from the tadalafil dose by several hours, and re-measure standing blood pressure after each addition. This combination is common because the same audience takes both.

  • Supplements affecting CYP3A4 — monitor. St. John’s wort induces CYP3A4 and reduces tadalafil exposure; grapefruit seed extract, bergamot and high-dose quercetin, resveratrol and berberine inhibit it and raise exposure. Mitigating action: avoid concurrent St. John’s wort, and treat a new CYP3A4-inhibiting supplement as equivalent to a dose increase.

  • Other interventions — caution. Sauna and hot bathing, alcohol, intense heat exposure, prolonged fasting and aggressive sodium restriction all lower blood pressure or reduce plasma volume and stack with tadalafil’s effect; the consequence is presyncope (light-headedness that stops just short of fainting). Combining tadalafil with another phosphodiesterase type 5 inhibitor, with intracavernosal alprostadil injection, or with testosterone replacement changes both the efficacy and the risk profile in ways no controlled study has characterized. Mitigating action: avoid sauna and heavy alcohol within a few hours of dosing.

  • Populations who should avoid tadalafil. Anyone taking any nitrate or riociguat in any form; anyone with recent myocardial infarction (within 90 days), recent stroke (within 6 months), life-threatening arrhythmia within 6 months, or NYHA Class III–IV heart failure; uncontrolled hypertension above 170/100 mmHg or hypotension below 90/50 mmHg; unstable angina (chest pain occurring at rest or worsening rapidly) or angina occurring during intercourse; severe hepatic impairment (Child-Pugh Class C); severe renal impairment (creatinine clearance below 30 mL/min) for continuous daily dosing; prior NAION in either eye; hereditary degenerative retinal disorders including retinitis pigmentosa; and anyone with an anatomical penile deformity or a condition predisposing to priapism such as sickle cell disease, multiple myeloma or leukemia. Tadalafil is not indicated in women outside pulmonary arterial hypertension, and is not indicated in anyone under 18.

Risk Mitigation Strategies

  • Low starting dose with slow titration: beginning at 2.5 mg once daily rather than 5 mg, holding for 7–14 days before any increase, and increasing only if there is no headache, reflux or myalgia, prevents the dose-dependent adverse effects that account for the great majority of discontinuations. Most people who abandon tadalafil do so in the first two weeks at a dose they never needed.

  • Bedtime dosing: dosing in the evening places the blood-pressure nadir and the peak vasodilatory headache during sleep, mitigating symptomatic hypotension, dizziness and fall risk as well as daytime headache. Because the half-life is 17.5 hours and steady state is reached in about five days, timing does not compromise efficacy on continuous dosing.
